Stone Med is an established medical clinic providing Primary Care, Low-Dose Pain Management/Joint Injections, Annual/Sports Physicals, Medical Weight Management, Botox® for Migraine & TMJ*, Hormone Balancing, and much more. With any treatments in the clinic, there are no guarantees or promises of improvement or recovery. Some treatments may not change your condition, and may, in rare instances, make your condition worse. Your provider will discuss all treatment options with you, including all risks and benefits associated with these treatments.
All Patients are encouraged to leave their valuables at home or with an accompanying family member or friend. Our facilities shall not be liable for the loss of or damage to any personal property including, but not limited to; money, credit cards, clothing, phones, other electronic devices, jewelry, glasses, dental devices, hearing aids, documents, or any other items brought into the clinic.
Our Facility is committed to providing all patients with high quality health care services, regardless of race, color, national origin, age, sex, sexuality, disability, or religious/political beliefs. The Health Insurance Portability and Accountability Act (HIPAA) requires that you read and sign the federally governed Health Care Privacy Notice. You may receive a free copy of this document. Your signature on the document confirms that you have read, understand and agree to comply with all terms and conditions of the Health Care Privacy Notice and all policies, consents, terms and conditions regarding your responsibilities to this Facility. You grant the providers and all staff of this Facility to use and share your confidential health information with others in order to treat you, and/or in order to arrange for payment of your bill and/or for issues that concern this Facility operations and responsibilities. Please direct any questions or concerns to a member of our office staff, if there are any questions or concerns.
Cancellations & Rescheduling
Same-day cancellations or rescheduling will result in a $25 cancellation fee. If you have to miss a scheduled appointment, please notify us at least 24 hours in advance. If you do not show up for your scheduled appointment, or "no-call no-show", two or more times you will be charged a $50 no-show fee that must be paid prior to being seen at your next appointment. No-show and cancellation fees will be added to your overall patient balance. As a courtesy to you, we may call when an appointment is missed to inquire why and/or to reschedule. If you do not wish for us to call you for appointment reminders, please let us know in writing and it will be added to your chart. Appointment reminders can also be sent via text message.
Insurance Benefits, Credit Policies,
This Facility will obtain a verification of insurance benefits as they are quoted to us. Some third-party payers misquote benefits, coverage and liability.
Our Facility is not responsible for what a third-party payer and/or representative may tell us. Our Facility will file initial insurance claims for you. Secondary claim submission and/or additional reports/documents sent for your benefit may result in an additional filing or medical report charges, which you are responsible to pay.
Co-pays, deductibles and all non-covered service charges are due the day the service is rendered. Patients are responsible for charges on all service(s) and/or product(s), which may exceed the maximum allowable and/or when a third party and/or insurance carrier does not reimburse this Facility enough to meet our cost of service.
All account balances must be paid in full within 90 days of treatment. Patients are fully responsible for all money owed to this office. Such payment is not contingent on any settlement, claim, judgment, or verdict by which they may eventually recover. It is also regardless of any attorney liens or pending settlement(s). If a third-party payer fails to pay said balance in full within the 90-day period, the patient must pay the balance in full. Assignee is fully responsible for all money owed this Facility for any and all treatment, products and services rendered to the patient or minor.
Returned checks, debit and credit charges made payable to this Facility for insufficient funds, stop payments or other reasons of non-payment will be assessed a $30.00 charge. For your convenience we accept most major credit and debit cards, as well as CareCredit.

All direct payment gateways offered by Wix.com and used by our company adhere to the standards set by PCI-DSS as managed by the PCI Security Standards Council, which is a joint effort of brands like Visa, MasterCard, American Express and Discover. PCI-DSS requirements help ensure the secure handling of credit card information by our store and its service providers.

We may contact you to notify you regarding your account, to troubleshoot problems with your account, to resolve a dispute, to collect fees or monies owed, to poll your opinions through surveys or questionnaires, to send updates about our company, or as otherwise necessary to contact you to enforce our User Agreement, applicable national laws, and any agreement we may have with you.
